As part of a team, you will support a staffing rota offering support to service users between the hours of 8am-10pm Monday - Sunday each week. \nIn collaboration with your team, you will actively monitor wellbeing, mental health, housing and living environment to ensure service users journey into independence. Moreover, you will take a solution focused and co-produced approach to supporting service users with their move-on into independent living.\nA day in this role looks like….\nAs a Support Worker at Hestia, you will play a crucial role in our Complex Needs service. Your contributions will directly influence the lives of our service users, guiding them towards a path of empowerment and independence.\nA day will never look the same and some of the things you can expect to do/be responsible for are:\n\nEmpower service users to engage effectively in their liaisons with professionals within the community such as community mental health teams, social work teams, housing providers, other health, employment and training professionals.\nAid other professionals in the continuous assessment of service users placements and support needs, advocating for service users when they need to step up or down into the appropriate support, and across their journey into independence.\nWork flexibly as part of a team to ensure service coverage and that contractual obligations are met.\nDevelop co-produced group activities, peer support networks and other community-based approaches to empowering service users to achieve their goals and aspirations and to encourage service users to access such opportunities\nYou will encourage service users to develop support networks in the local community and help them engage in meaningful activities (including managing volunteers and befrienders for your caseload).\nEnsure referrals, assessments and service reviews are effective and timely.\nActively monitor the health, wellbeing, safety and security of the service and its users ensuring all concerns are followed up promptly and in line with procedures.\nParticipate in the core work of the service, providing support to colleagues, carrying out tasks allocated by the management team and using initiative to identify and deal with additional tasks as they arise.\nEnsure that administration is done in good time; that detailed, concise and accurate records are created contemporaneously and that confidentiality is appropriately maintained\nCarry out duties, in line with organisational policies and procedures\nEmpower a caseload of clients to manage their health and wellbeing, maintain their independence, identify their strengths and achieve their goals and aspirations.\nEmpower service users to develop independent living skills as part of a planned move on process.\nMake best use of property, equipment and information technology. \n\nWhy Hestia?\nJoining Hestia means more than just a job; it's an opportunity to be part of something bigger. Our mission is to empower individuals to rebuild their lives and achieve independence.\nAbout the service....\nThe service you will work on is Hammersmith and Fulham Mental Health Accommodation Service in South Cluster, which consists of 3 services at Edgar Wright Court, Moore Park Road and Bishop Road. We support people in enduring mental health crises aged 18+ who have housing and support needs. They may be leaving the hospital, being registered for care, being homeless, being threatened to be homeless or living accommodation that is not meeting their identified needs. The services will also be used as a ‘step up' or ‘step down' from other supported accommodation, where appropriate.\nA day in this role looks like…..\nAs a support worker at Hestia, you will play a crucial role in our Mental Health Service.
